Waiver

ACKNOWLEDGEMENT OF RISK, WAIVER OF LIABILITY, INDEMNIFICATION, AND CONSENT  

I hereby consent to participation in Laws of Strength programs, including, but not limited to, competition, training and practice.   These programs include but are not limited to in person, online and/or virtual participation. I recognize that potentially severe injuries can occur in any activity involving height and/or motion, weight lifting, physical activity and other related activities. It is recommended that a physical be performed by a licensed medical doctor prior to participation in the aforementioned activities.  By virtue of the execution of this document, I hereby affirmatively state that I am physically able to participate in any and all Laws of Strength activities.

READ THIS FORM COMPLETELY AND CAREFULLY. YOU ARE AGREEING TO ENGAGE IN A POTENTIALLY DANGEROUS ACTIVITY. YOU ARE AGREEING THAT, EVEN IF LAWS OF STRENGTH USES REASONABLE CARE IN PROVIDING THIS ACTIVITY, THERE IS A CHANCE YOU MAY BE SERIOUSLY INJURED OR KILLED BY PARTICIPATING IN THIS ACTIVITY BECAUSE THERE ARE CERTAIN DANGERS INHERENT IN THE ACTIVITY WHICH CANNOT BE AVOIDED OR ELIMINATED. BY SIGNING THIS FORM, YOU ARE GIVING UP YOUR RIGHT TO RECOVER FROM LAWS OF STRENGTH IN A LAWSUIT FOR ANY PERSONAL INJURY, INCLUDING DEATH, OR ANY PROPERTY DAMAGE THAT RESULTS FROM THE RISKS THAT ARE A NATURAL PART OF THE ACTIVITY. YOU HAVE THE RIGHT TO REFUSE TO SIGN THIS FORM, AND LAWS OF STRENGTH HAS THE RIGHT TO REFUSE TO LET YOU PARTICIPATE IF YOU DO NOT SIGN THIS FORM.  

In consideration for allowing me to use the facilities used in workout related activities, as well as other locations, where Laws of Strength practice, competition, and any other use, as designated by Laws of Strength, may occur, I hereby forever release Laws of Strength, its employees, instructors, coaches, agents, and any other volunteers, from any and all liability, for any and all damages and/or injuries, suffered by me while under the instruction, supervision, or control, direct or indirect, of Laws of Strength, its employees, instructors, coaches, and agents, and any and all other volunteers, whether caused by their own negligence or otherwise.  I hereby acknowledge that it is my sole responsibility to provide for any medical expenses or other attendant costs which may be incurred by me as a result of any injury sustained while under the instruction of Laws of Strength, its employees, instructors, coaches, agents, and any and all other volunteers. In recognition of the execution of this Acknowledgment of Risk, Waiver of Liability,

Indemnification, and Consent, I further agree to indemnify Laws of Strength, its employees, instructors, coaches, agents, and any and all other volunteers, from any claim for damage or injury of myself, including reasonable attorney’s fees incurred in defending any such claim.    This Acknowledgment of Risk, Waiver of Liability, Indemnification, and Consent, having been read and understood completely, is signed voluntarily as to its content and intent. This Acknowledgment of Risk, Waiver of Liability, Indemnification, and Consent shall continue in full force and effect until such time as revoked, in writing, by the undersigned, without limitation as to its duration. If signing for a minor, the undersigned further acknowledges that he/she is authorized to execute this Acknowledgment of Risk, Waiver of Liability, Indemnification, and Consent on behalf of the child and the parents, guardian, legal representative, personal representative, assigns, heirs, and next of kin.  

The undersigned further acknowledges and agrees that Laws of Strength shall be granted specific permission to use my image and/or likeness for any purpose as deemed applicable by Laws of Strength, including photographs and/or videotape, and to further use my name, face, likeness, voice, and appearance in connection with exhibits, publicity, advertising, and promotional materials without reservation or limitation as to the duration of the consent.

The undersigned expressly agrees that this Acknowledgment of Risk, Waiver of Liability, Indemnification, and Consent is intended to be as broad and inclusive as permitted by the laws of the State of Florida, and that if any portion of this document is held invalid, it is agreed that the valid portion/s shall, notwithstanding, continue, in full legal force and effect. I authorize Laws of Strength to retain and rely upon any document executed including but not limited to the Release in electronic form and shall not challenge its authenticity in the event the original is destroyed after electronic preservation.

In order to avoid any potential for confusion as to the scope and terms of our agreement, you and Laws of Strength agree that this written document contains all the agreements, promises and representations that you and Laws of Strength are relying upon in entering into this agreement. This document represents the parties’ entire agreement and there are no changes or additions to the typewritten portions except for the filled-in blanks, unless they are initialed by both parties at each place a change is made or are included in an attached written addendum signed by both parties.
